REQUIREMENTS

-Documentation: An original document from the place of birth must be submitted as proof of the applicant’s name and age. The name, DOB, signature and picture on the application must be the same as the information stated in the original document. (Applicants of the states of Florida and South Carolina see attached memo for the process of a FL & SC application.)

 

Samples of Acceptable documents:

• Passport
• Driver’s License
• Birth Certificate
• Any form of governmental document of applicant’s country of birth stating applicant’s personal information.
  
  

 

 

-Photo: A color photo of the applicant, passport type or digital on a blank background, facing front, without glasses nor hat. (See sample of photo accepted)

 

-Signature: The applicant’s signature must be uploaded up loaded. You can take a photo of your signature using a digital camera or a cell phone. (Use black ink for better results on the id) (See sample of photo accepted)

 

 

*For Minors: If the applicant is a minor, the parent or guardian of a minor applicant must submit in his/her signature to authorize the id. (Parent’s/guardian’s signature will not appear on the minor’s id, only the signature of the minor who’s able to sign will be on the id.)

 

 

-3 ids styles: Choose one of our id-card designs, if none is chosen, the classic style card will be made automatically. (Note: there is a fee to change to another style)

La foto, firma y nombre deberán ser iguales a la información del documento original. (Solicitantes de los Estados de Florida y Carolina del Sur ver memorando incluido antes de procesar una solicitud de FL y SC.)

 

 

Ejemplos de Documentos Aceptables:

 

 

– Pasaporte

– Licencia de Conducir

– Cedula

– Tarjeta Electoral

– Certificado de Nacimiento

– Documento Gubernamental del país de origen del solicitante demostrando su información personal.

 

 

-Foto:Una fotografía del solicitante a color, tipo pasaporte o digital de frente, sin gafas ni sombrero y fondo blanco,(Ver ejemplo).

–Firma: La firma del solicitante debe ser subministrada, Puede tomarle la foto a su firma con una cámara digital o su celular.(Usar tinta negra al firmar para mejor resultado en el id) (Ver ejemplo)

*Para menores de edad: Si el solicitante es un menor de edad, deberá tener asistencia de su padre o guardián, este deberá ingresar su firma para para autorizar el proceso del el id. (La firma del padre o del guardián no aparecerá en el id, solamente la firma del menor siempre y cuando pueda firmar.)

 

 

 

 

-3 estilos de Id-Card: Escoja uno de los tres estilos de ids, de no hacerlo se hará el estilo clásico. (Nota: después habrá un costo para cambiarlo)

 

Por favor revise la información suministrada antes de enviar para evitar futuros reclamos y costos adicionales.

 

Atencion: AmeraCard NO procesará su Id si no cumple, con los requisitos anteriormente mencionados o si tenemos sospechas de fraude. (Nosotros le notificaremos por email y se les hará un reembolso)
